Adult Player Deletion/Move Form

Deletions will not be accepted past the deadline.
Amount refunded is $15 for all leagues except Trio and Flex. Trio refund is $10. Flex refund is $12. All refunds are sent via check.
Instead of requesting a refund, players have the option to be moved to a different league during the current calendar year and avoid any loss of funds. If you wish to use this option, please submit a “move form” instead of a deletion form. Trio registrations cannot be moved to a different team.
